Reports show that a large number of hospital readmissions are caused by patients who do not get their prescriptions filled, or do not take their medications properly. St.Louis Hospital had implemented a program that includes prescription counseling and discharge planning with patients as well as the option to fill their prescriptions before they leave the hospital. Because of this, they have seen an increase in patient compliance. At the Nebraska Medical Center, readmission rates dropped 8% as a result of medication education from pharmacists.
