Sussex County is the first government agency to implement a hospital readmission prevention program. The program functions by assigning case workers to each discharged patient, in order to ensure that they are receiving the follow up care that they need. Because of this attentive care, patients are able to address problems before they escalate into an emergency room visit. Since the program’s implementation, less than 1% of participants have been readmitted into the hospital for the same condition within 30 days of discharge.
