A coalition of non-profits and health care agencies in California has begun a website to help curb hospital readmissions. The website organizes the services that are available in the area to older adults who have been recently discharged. After filling out a form, the user can identify which services would be best for them. The article identified the YWCA and YMCA pools as two resources that are under utilized by older adults.
